Els forms for all services Via Oberta they are already available in pdf format on the AOC Consortium website.

What changes are there?
The fields have been unified to the maximum taking into account the particularities of each service. In each associated mode, two drop-downs have been added, one that includes the purposes associated with the services and the other that opens a new window for requesting more than one purpose within the same service. The opening of a window by ticking NO in the consent box has also been added in order to indicate the legal regulations that exempt it from being collected.
